Utah State Epidemiologist Dr. Leisha Nolen and retired Canadian Public Health Agency virologist Dr. Alberto Severini reviewed the sequencing and reached the same read: a single virus, moving continuously through at least 16 states, never interrupted long enough to die out and need reimporting. Twelve letters out of sixteen thousand is a small number because measles mutates slowly and predictably enough that virologists can use it as a clock: no gap in the sequence means no gap in the chain.
PAHO's own definition is procedural, not a judgment call: a same-genotype chain running domestically, uninterrupted, for twelve months or more constitutes reestablished endemic transmission, full stop. The Texas-to-Utah chain crossed that mark around the middle of 2026, by the timeline ProPublica's sequencing lays out. What has not happened yet is the paperwork. Noel T. Brewer chairs the US National Verification Committee, reviewing the evidence through August 2026, with findings passed to PAHO's Regional Verification Commission for a decision due in November. CDC's own tally, 2,777 confirmed cases as of August 20, already exceeds the 1,274 cases that made 2019 the worst year since 1992, and 93 percent of this outbreak's patients were unvaccinated or of unknown vaccination status, per Johns Hopkins' International Vaccine Access Center. State health departments across the sixteen states along this chain have spent the summer running exposure tracing and vaccination pushes under a federal label, 'eliminated,' that the government's own sequencing data says is already wrong. Hong Kong, logging 9 cases in the same period per the Centre for Health Protection, has not had that problem. An agency that certifies disease elimination on an annual lag will keep rediscovering, every year, that the lag exists.
November's announcement will read like news. It is closer to a coroner's report, dating a death that already happened. The more interesting question is what changes when the label finally falls: does funding shift, does a single school district alter its outreach because Washington caught up to what Utah's state epidemiologist already knew in June.
